A source receipt or selected calculation was checked during the review, and a named human reviewer approved the status. Both conditions are required.
The method keeps source-checked facts separate from interpretation, assumptions, unsupported wording, contradictions, and matters that remain unknown. Software may assist the work; it cannot issue the final client conclusion.
A provisional label describes the relationship between a statement and the material available at that moment. It is not a legal finding, a character judgment, or a prediction of what another decision-maker will conclude.

A reasonable conclusion from supported facts, but not something the checked source establishes directly.
A gap filled without direct evidence. The assumption and its effect on the statement are made visible.
The statement was presented as fact without enough support in the material checked. This does not mean the statement is proven false.
Two or more supplied items conflict on a decision-sensitive point. The conflict remains visible until it is resolved.
The matter cannot be established from the available material, or the relevant source was outside the agreed scope.
A link, filename, founder statement, management summary, publication identifier, or model output does not prove a claim by itself. The report records what was actually opened or received, which version was checked, the relevant excerpt or calculation, and the limitations that remained.
Not legal advice: when the important question requires a legal or other licensed conclusion, the report identifies the focused question and stops at the boundary. A licensed attorney or other qualified professional must provide that conclusion.
The sample shows source records, provisional evidence statuses, contradictions, narrower wording, and the human sign-off block.
